MGH to look for causes of “hand-off” errors.

The Joint Commission, which accredits hospitals, has chosen ten site,  including Mass General, “to find the causes of and put a stop to dangerous and potentially deadly breakdowns in patient care when responsibility for patients is transferred or handed-off from one caregiver to the next”

An estimated 80 percent of serious medical errors involve miscommunication between caregivers when responsibility for patients is transferred or handed-off from one caregiver to another. Using Robust Process Improvement™, the Center and the participating hospitals discovered barriers to effective hand-offs and developed targeted solutions. Using solutions targeted to the specific causes of an inadequate hand-off, participating organizations that fully implemented the solutions achieved an average 52 percent reduction in defective hand-offs.

Nothing beyond that on the group’s website but details expected at a Thursday press conference.
